CANCEL is only sent from UAC to UAS and any non-2xx response can be used
by UAS to reject the call from the Calling Party (UAC).

CANCEL is a request, not response.
If the call is not confirmed, callee plays a role of UAS.  
It MUST NOT send CANCEL request to terminate the session.
And 4xx response may be a good choice.
