1.There has to be some event to which the client1 and client2 have to be subscribed to.(SUBSCRIBE with some event for handover)2.once that event (let us name it handover) triggers , a NOTIFY should come from core network3. re-INVITE or UPDATE should go from client1 towards core stating the SDP changes , bandwidth changes etc4.Once that is accepted by core then the media bridge should resume( what sakharam has put , a network hold and subsequently a resume)5. feature can be anchored on a SBC or Softswitch ( acting as a media anchor)

How is the "handover" actually done?  
You may use SIP re-INVITE method. when network change is detected, Put client 2 on-hold; send reinvite, then unhold client 2.

How does a SIP client handle handover seamlessly (from the user's point-of-view) when the user is migrating from one network to another while the SIP client is in an active SIP call?

Here's how I imagine it:

1. Client 1 is on a wifi network and assigned a public IP address IP1
2. Client 2 is on some network and assigned a public IP address IP2
3. RTP data is sent from IP1 to IP2
4. RTP data is sent from IP2 to IP1
5. Client 1 migrates from the WIFI network to a cellular network and is assigned a new IP address denoted IP3
6. Client 2 is somehow notified about client1's migration 
3. RTP data is now sent from IP3 to IP2
4. RTP data is now sent from IP2 to IP3

I'm sure that there'll be some kind of minor packet loss on both endpoints during migration ?

How is that packet loss minimized?

How is the "handover" actually done? 

Is there some kind of protocol for a handover?
