Are you talking about TLS enablement in a SIP network ?
Once the secure connection is established between each and every hop
between Sender and Receiver UA/C, SIPS URI is used for secure signaling.

For media, encryption mechanism or secure RTP like zRTP etc. are used.

RFC 3261 talks in detail about proxy/UA behavior Section 8.1.1.2 and
also section 26 (also 26.2) talks in details about security issues and
SIPS URI scheme.
