And, answering the original question, there absolutely is no RFC describing when to charge, nor will there ever be. The IETF is about getting the endpoints to work. The IETF is not about business models.

This is very important to understand, because it means in an absolute sense there is no right time to start or end billing. All of the suggestions you have received on this list are very informed suggestions. The 3GPP document is the law for IMS implementations, but not for anything else.
