There are a number of avenues you can try:

Microsoft RTC:  http://msdn2.microsoft.com/en-us/library/ms775893.aspx

If you are using C/C++, you can try the SipFoundry stuff:
http://sipfoundry.org
    sipXtackLib is their SIP stack library
    sipXcallLib has a call manager and has integrated media
    sipXtapi is a high level user-agent api

If you are using Java, the best bet is jain-sip:
http://jain-sip.dev.java.net/

If you are using Java, and want a higher level api, with media, take a look
at flibble-voip:  http://code.google.com/p/flibble-voip

These are the only ones I have personal experience with, but, I'm sure there
are other choices out there...
