Many ISPs provide SIP service. Here in NA, Telus is one of the SIP
service providers. Actually the underlying technology is transparent
to the end user be i SIP or H.323 or something else. For a good start,
i suggest you to visit www.iptel.org, they provide an open source SIP
express router and that would clarify many things.
